Loving Hearts Hospice and Palliative Care offers a compassionate team dedicated to helping you receive care, remain at home, and focus on quality of life.   • Individualized care plan for the patient which includes physical, spiritual, and emotional needs

  • Quality of life focused to help patients stay independent longer and spend less time in the hospital or emergency room visits

  • Home visits from our team members including specialized care and volunteers

  • Provides medication and all medical equipment related to the hospice diagnosis with no out of pocket expenses for the patient.

  • Support to family during the time of care and support through the grief process

How to Get Started

Anyone can refer someone to hospice care or request information. With permission from the patient and/or family, Loving Hearts Hospice will handle gathering the information needed to obtain an order for hospice. All hospice services are covered 100% by Medicare, Medicaid, and by most private insurances. Covered costs include care, medications, and needed medical equipment.
